Why Is This Important?

Kidney failure treated with dialysis or a kidney transplant is called end-stage kidney disease (ESKD). Diabetes is the most common cause of ESRD.[https://www.cdc.gov/kidneydisease/publications-resources/2019-national-facts.html ^1^]

Healthy People Objective: Reduce the rate of new cases of end-stage renal disease (ESRD)

U.S. Target: 34.4 new cases per 100,000 population
State Target: 33.8 new cases per 100,000 population

Other Objectives
